Office Hours and Appointments

Our normal office hours are from 8:30 a.m. to 5:00 p.m., Monday through Friday. The office hours and locations vary for each doctor each day, so please call our main office at (920) 965-7411 or (866) 965-7411 to schedule an appointment.

Please dial 911 for medical emergencies. If you have a question after hours you may contact our physician on call anytime by dialing (920) 965-7411.


Your First Appointment

A pre-registration specialist will call you before your appointment to complete the necessary patient information forms. If you would like to contact a pre-registration specialist at a time that works well for you, please dial (920) 965-4716 or (920) 965-4722.

Please bring your insurance card, referral form (if needed by your managed care company), imaging and reports, a list of current medications and dosages and any other medical information or records that may help our doctors better understand your condition and prior treatment.


Prescription Refills

Please call for prescription refills during regular office hours, 8:30 a.m. to 5:00 p.m., Monday through Friday. Please have the prescription name and dosage and the pharmacy name and telephone number available when you call. Please remember we do not refill narcotics or any other pain medication after 3:00 p.m. weekdays or on the weekends.

If you should require a surgical procedure in the hospital, as a courtesy to our patients, our office pre-certifies all surgical procedures with your insurance. This certification does not guarantee payment. We urge patients to contact their insurance companies prior to procedures.


Payment for Services and Fees

The general fees charged by this office are comparable to those charged by other competent general and vascular surgeons in this area. You are requested to pay for office visits and treatments at the time the service is rendered. Your personal check, cash, MasterCard, Visa and insurance card are accepted. If you have a question about the fee for a specific visit or service, please contact our billing office at (920) 430-8120.


Billing and Insurance
An itemized statement covering all medical services will be mailed to you on a monthly basis. If you are hospitalized or surgery is performed, you will receive separate bills from the hospital and from other physicians who might be involved in your care.

As a courtesy to our patients, we will submit claims to your insurance company on your behalf. Please remember, your insurance policy is a contract between you and your insurance company and it is important that you understand its provisions. Of course, we cannot guarantee payment for your claim. If your insurance company pays only a portion of the bill or rejects the claim entirely, this does not relieve you of the financial obligation you have incurred.

Many insurance plans require deductibles or co-payments at the time of the visit. You may pay these with cash, check, VISA or MasterCard.

Medical Records

You are welcome to request copies of your medical records by completing the Patient-Identifiable Health Information form located under the forms section of this website and mailing or faxing to our office. A fee for records is assessed under most situations. Please contact our office at (920) 965-4716 if you have further questions.
